    NHIF is gone.

Well, technically it still exists as an organization. But the old NHIF you knew — the one where you paid KES 500 flat — that's finished.

SHA and SHIF have replaced it.

And if you're still confused about what changed, you're not alone. Here's the complete breakdown.

    What Actually Changed

    1. Contribution Structure

    Before (NHIF):

    • Self-employed: KES 500/month flat
    • Employed: KES 500–1,700/month based on income bands

    Now (SHIF):

    • Everyone: 2.75% of gross income
    • No minimum or maximum (though effectively there are operational floors)

Example: If you earn KES 50,000/month:

    • Old NHIF: ~KES 800/month
    • New SHIF: KES 1,375/month (2.75% × 50,000)

    For many people, contributions increased.

    2. Benefits Package

    Before (NHIF):

    • Inpatient: yes
    • Outpatient: limited rebate system
    • Maternity: yes
    • Chronic: basic coverage

    Now (SHA):

    • Inpatient: expanded
    • Outpatient: direct cover (not rebate)
    • Maternity: enhanced
    • Chronic: better management programs
    • Mental health: included
    • Emergency: ambulance covered

The benefits package is genuinely better — if you can access it.

    3. Management Structure

    Before:

    • Single entity: NHIF

    Now:

    • SHA (Social Health Authority): Sets policy, accredits facilities, manages system
    • SHIF (Social Health Insurance Fund): Holds and manages contributions
    • Separation of powers: Policy vs. money management split

    4. Hospital Network

    Before:

    • NHIF-contracted facilities

    Now:

    • SHA-accredited facilities
    • Supposed to include more private hospitals
    • Still expanding

    What Stayed the Same

    • Mandatory: Both NHIF and SHIF are compulsory for formal sector employees
    • Employer deduction: Still deducted from salary automatically
    • Family coverage: Spouse and children covered under your contribution
    • Public hospitals: Still accessible
    • Basic benefits: Core health coverage remains

    Did Your NHIF Record Transfer?

    This is the big question.

The answer: It should have, but not everyone's did.

If You Were an NHIF Member

Your records were supposed to migrate automatically to SHA/SHIF. Check your status:

    1. Download SHA Super App
    2. Login with your ID number
    3. Check if your details appear
    4. Verify dependents are listed

    If Your Record Is Missing

    1. Call SHA: 0800 720 500
    2. Have your old NHIF number ready
    3. Provide ID and phone number
    4. They'll trace and migrate your record

    For Employers

Verify all employees appear in the new system. Use the SHA Employer Portal.

    Contributions: The New Math

    For Employed Kenyans

Your employer calculates and deducts 2.75% from your gross salary.

    Monthly SalaryOld NHIFNew SHIFChange
    KES 20,000KES 600KES 550-8%
    KES 50,000KES 800KES 1,375+72%

    KES 300,000KES 1,700KES 8,250+385%

Observation: Low earners pay about the same or less. Middle and high earners pay significantly more.

    For Self-Employed Kenyans

Declare your income and pay 2.75%.

Minimum contribution: Approximately KES 300/month for low-income earners (this is operationally enforced even if 2.75% of income would be less).

    What About Your NHIF Card?

Your old NHIF card is being phased out.

    Now:

    • Verification is done via National ID
    • The SHA Super App provides digital verification
    • Hospitals check the SHA system, not your card

Keep your NHIF card for now — some facilities still reference it during transition.

    Common Transition Issues

Problem 1: "My Contributions Were Deducted But I'm Inactive"

This happens during transition periods. The money is taken but the system hasn't caught up.

    Solution:

    1. Keep your pay slips
    2. Report to HR
    3. Call SHA with documentation
    4. They'll verify and activate

Problem 2: "My Dependents Aren't Showing"

Dependent data didn't always transfer correctly.

    Solution:

    1. Open SHA Super App
    2. Go to "Add Dependent"
    3. Submit documents (marriage cert, birth certs)
    4. Wait 3–7 days for approval

Problem 3: "The Hospital Says They Don't Take SHA"

Some hospitals are still getting accredited or having system issues.

    Solution:

    1. Verify on SHA app that hospital is accredited
    2. Show the app to hospital admin
    3. If still issues, call SHA
    4. Pay and claim reimbursement as last resort

Problem 4: "I Was Paying NHIF as Self-Employed and Now It's More Expensive"

Yes, this is by design. The new system is income-based.

    Options:

    • Continue paying the new rate
    • If income is genuinely low, you may qualify for lower contributions
    • Consider supplementing with private insurance for better service

    Employer Transition Checklist

If you're an HR manager or business owner:

    • [ ] Register on SHA Employer Portal
    • [ ] Verify all employees appear correctly
    • [ ] Update payroll to deduct 2.75%
    • [ ] Submit monthly contributions by the 9th
    • [ ] Handle employee queries about the new system
    • [ ] Keep records of all submissions

    What If You're Self-Employed?

    Registration

If you weren't an NHIF member:

    1. Download SHA Super App
    2. Tap "Register"
    3. Select "Self-Employed"
    4. Declare income
    5. Pay first contribution via M-Pesa

    Ongoing Payments

    • Due by 9th of each month
    • Pay via M-Pesa (Paybill 222222, Account: Your ID number)
    • Late payments attract penalties
    • 30-day waiting period before using benefits

Should You Get Private Insurance Too?

SHA covers basics. Private insurance adds:

    • Private hospital rooms
    • Choice of doctor
    • Shorter wait times
    • Better dental and optical
    • International coverage

If you can afford it, private cover on top of SHA is ideal.

Is NHIF completely gone? NHIF as an organization still exists but is being restructured. The NHIF fund and contribution system you knew is replaced by SHIF under SHA management.

Can I opt out of SHA/SHIF? No. It's mandatory for all employed and self-employed Kenyans.

What if I was behind on NHIF payments? Arrears may need to be cleared. Contact SHA to discuss your specific situation.

Do I need a new card? No physical card required. Verification is via National ID and SHA app.

Are all hospitals accepting SHA? Not yet. The network is expanding. Always verify before visiting.
